A standard Video Release Form is a document that grants permission to a third party to utilize the voice and appearance of an individual in both videos and still images for the advantage of a said third party. Such advantages can include using the materials for media, online, physical products (such as DVDs or CDs), or any other publication. The form must also include a section about what the agreed-upon payment will be by the production company that is to be checked by the subject completing the form.
